Conditions and Requirements
- The request shall be submitted by the author in person, by the rights holder or agent thereof, under duly notarized documents.

- Documents indicating infringement
- Documents proving the applicant's ownership of the intellectual works or his client’s ownership of the intellectual works that are the subject of the complaint
